Attendance Forms

Please report all absences to the attendance office by 8:00am by filling out the Report an Absence form below.    It is critical to student safety that we know where all students are on school days.  Please notify the school each day your child will be gone from school. Office staff will place calls and/or send text messages daily to parents for all unreported absences.

High School Permission to Drive Form

The High School Student Permission to drive form is valid until your student either leaves Caprock Academy or graduates.  New drivers must complete and submit a Permission to Drive Form on or after July 1, or as soon as they begin driving on and off campus.  Please note that these forms will be valid until parents or guardians revoke or change the information.

Federal Economic Data Survey Forms

Completing the annual FEDS Forms helps determine eligibility for hot lunch and sports scholarships, helps us complete required reports to Colorado Department of Education (CDE), and may allow for additional funding for Caprock Academy.  It takes around 5 minutes to complete, and you receive 2 full volunteer hours for completing the form.  If you have any question or concerns about completing the FEDS form, please contact Heidi Kellogg h.kellogg@caprockacademy.org.

Special Note: All FEDS forms must be turned in to your child's teacher or to the front desk in an envelope to protect your privacy, they will not be accepted electronically via email or fax. 

 

Student Scholarship Request Form

Families in need of financial assistance must complete a Family Economic Survey prior to their request.  All financial assistance requests must be requested prior to the start of each activity or fee and must be completed each school year, on or after July 1st.  FEDS forms must be completed and submitted prior to completing this request.

Community Requests for Facility Use and Fundraising Requests

Caprock Academy welcomes community use of our campus. Use the forms below to request facilities for meetings, events, or special needs; to seek approval for fundraisers benefiting school groups (e.g., NHS, Student Council, Athletics, PTO); or to request planning support if you need help brainstorming, exploring options, or coordinating logistics. All requests are subject to approval by administration.
